COLLECTIVE REVIEWS Management of Recurrent Inguinal Hernias
نویسندگان
چکیده
The ultimate measure of success of inguinal hernia repair is the rate of recurrence. Although other procedure-related complications are important and have been shown to affect healthrelated quality-of-life parameters, recurrence is more challenging for the patients and the surgeon. From the patients’ perspective, the initial physical and mental investment in the operation has failed. A new investment has to be made, with fewer guarantees for success, more risks for more serious complications, and more time off from work. For the surgeon, repair of a recurrent inguinal hernia is technically more demanding because scar tissue causes the inguinal canal to be obscured and distorted. In addition, the tissue tends to be weaker than at the time of primary repair, resulting in a substantially higher risk for complications or development of another recurrence. Although much has been published about primary repair of inguinal hernias, less is known about the best approach to address a recurrent hernia. In this review, we address the “pros and cons” of popular surgical approaches and watchful waiting for recurrent inguinal hernias and indications for each. In the absence of best evidence in this field, the authors present an opinion based on their experience and a review of the available literature primarily extracted from large trials and large cohorts for inguinal hernia repair in general.

BACKGROUND Gender differences are expected to influence the pattern and outcome of management of abdominal wall hernias. Some of these are left to speculations with few published articles on hernias in females. OBJECTIVES To describe the clinical pattern of abdominal wall hernias in females. METHOD A 5 year retrospective review. RESULT There were 181 female patients with 184 hernias repre...
متن کاملCure of inguinal hernias with large preperitoneal prosthesis: Experience of 2,312 cases
It is clear that the recurrence rates after nonprosthetic methods for the repair of inguinal hernias, like McVay, Bassini or Shouldice techniques, are high (6-10%). Since 20 years, we are convinced, in the GREPA-EHS group, about the advantages of the use of a prosthetic mesh in majority of patients for repairs of primary or recurrent inguinal hernias and incisional hernias. We describe our typi...
متن کاملPatterns of Recurrent Hernias according to the Types of Previous Hernioplasties
Purpose: Recently, conventional tissue repairs are gradually being replaced by tension-free hernioplasties using meshes (mesh repairs) in hernia surgery. The aim of the present study was to evaluate patterns of recurrent hernias according to the types of previous hernioplasties. Methods: From August 2005 to July 2008, 18 hernioplasties were performed in recurrent cases at Chungbuk National Univ...
متن کاملChanges of important anatomical structures in the inguinal region after a herniorrhaphy: observations during treatment of recurrent hernia using TEP
In this study we will describe important anatomical structures in the inguinal region as well as their changes after a herniorrhaphy, as they were seen during treatment of 135 recurrent hernias between 1993 and 2003 using TEP. We observed in most cases an upwardly curving arc-shaped deflection of the inguinal ligament and ileopubic tract, which may be due to tension caused by the attachment of ...
متن کاملEfficacy of the open anterior prosthetic repair in the surgical management of recurrent inguinal hernia in old patients
Background To date no standard treatment for recurrent ingunal hernias exists. Open or laparoscopic approaches were proposed with apposition of meshes with reknown advantages and limits [1]. Long term recurrence-rate, postoperative pain, fistulization or prosthetic infection risks remain open questions in this surgery. Aim of this work is to show results of our experience with the open anterior...
